What is the best way to tell if the ai worked? If she does not come back into heat in 21days is she bred then?

The Captain Obvious answer is that you'll see pigs on the ground about 114 days later! But seriously, one thing you want to watch is to see if she swells and cycles 20 to 21 days after she was in heat. If she does, then you missed, if not then you might be ok. Unfortunately, if there is only a small number of conceived embryos, the female might sluff the litter around 24 to 28 days. So you need to continue to watch for heats all the way past 30 days bred. On gilts, they will start to show that "show fill" look at around 30 days bred. This is another good sign to watch for. If you are only feeding 5 pounds of gestation feed a day (optimum amount of nutrition for a gestating sow), she won't look full because of the feed. typically, that full look will be from growing fetus's and the placenta!

is it when she is in standing heat or just when she starts swelling

I base our time clock on when she was locked up in heat. However, if you have one that swells a few days before she will stand in heat, you should watch for swelling. If you start to see swelling, then you need to order more semen.
